What is the 5-year success rate of a corneal transplant?
The short answer? 76%.
A new study published in the Canadian Journal of Ophthalmology examined 596 patients who had primary penetrating keratoplasty between 2011 and 2017. The authors found the 3-year survival rate was 78%. At 5 years, the survival rate was 76%, and at 7 years it was 73%.
Is this different from what was previously published?
Yes. Previous studies that the success rates were higher, around 90% at 5 years.

Can glasses protect me from COVID? A recent study published in JAMA Ophthalmology found that glasses wearers may be less likely to be infected with COVID-19.
Tell me about the study.
The study examined 276 patients in Suizhou, China, who were hospitalized due to COVID-19. Of these patients, 30 wore glasses (16 were myopic and 14 were presbyopic). The 16 myopic patients with COVID-19 who wore their glasses for more than 8 hours per day accounted for 5.8% of patients in the study.
Why is this significant?
One of the main findings of this study was that patients with COVID-19 who wear glasses for more than 8 hours per day were relatively uncommon, representing only 5.8% of patients. The authors associated this with less susceptibility to COVID-19 infection.
Why do they think wearing glasses helps?
There are a couple of theories:
1. Wearing glasses prevents or discourages wearers from touching their eyes, thus avoiding transferring the virus from the hands to the eyes.
2. An abundance of the SARS-CoV-2 receptor angiotensin-converting enzyme 2 has been found on the ocular surface through which SARS-CoV-2 can enter the body. Wearing glasses may become a protective factor, reducing the risk of virus transfer to the eyes.
Why might my patient ask me about this?
Because this study made its way into mainstream media (here, here, here, and here, to name a few sources).
Should we recommend that everyone wear glasses?
Not yet. In fact, an infectious disease specialist at Johns Hopkins School of Medicine wrote a commentary on this study discussing some of its flaws.

Gilead Sciences said Monday that it hopes to begin clinical trials this summer evaluating an inhaled formulation of remdesivir, administered via a nebuliser, to patients with COVID-19. Does COVID-19 affect the retina?
A small study published in The Lancet suggests that it does.
Tell me about the study.
Researchers used OCT to evaluate patients infected with SARS-CoV-2 to determine whether subclinical retinal changes were present. A total of 12 patients were examined 11 to 33 days after the onset of symptoms. All patients had normal blood parameters at the time of their eye exam.
What did they find?
All patients showed hyperreflective lesions at the level of ganglion cell and inner plexiform layers, more prominently at the papillomacular bundle in both eyes. Also of note, four patients presented with subtle cotton wool spots and microhemorrhages along the retinal arcade. The researchers noted that visual acuity and pupillary reflexes were normal in all eyes and that no symptoms or signs of intraocular inflammation were detected.
The take home:
The study authors stated that although COVID-19 animal models suggest that ocular lesions could include retinitis and optic neuritis, this is, to the best of their knowledge, the first report of retinal findings possibly associated with COVID-19 infection in humans.

The Future of Limbal Stem Cell Transplantation
In her review of limbal stem cell transplantation techniques and surgical outcomes, Ashiyana Nariani, MD, MPH, highlights recent scientific work aiming to address some of the issues associated with current standards of care. Among the advances are several modified versions of simple limbal epithelial transplantation and xenofree alternatives for culturing limbal epithelial stem cells.
Predicting Graft Survival After PKP
Eric J. Shiuey, MS, reports on the use of a novel nomogram to predict graft survival after penetrating keratoplasty (PKP) and shares the top six variables with significant predictability of graft failure. Mr. Shiuey and colleagues at Wills Eye Hospital in Philadelphia conducted a study of more than 1,000 corneal transplants performed since 2007.
A Deep-Learning Neural Network for Diagnosing Corneal Diseases
Use of a multi-disease, deep-learning neural network algorithm can simplify the diagnosis of common corneal diseases, Mohamed Abou Shousha, MD, FRCS, PhD, reports. He and researchers from Bascom Palmer Eye Institute in Miami developed a device to autonomously diagnose dry eye disease, keratoconus, and Fuchs endothelial dystrophy. In an independent validation study, their AI algorithm successfully classified 100% of Fuchs endothelial dystrophy, 97% of keratoconus, 90% of dry eye disease, and 92% of healthy eyes.
