Learning Collaborative Opportunity: Non-Clinical Diabetes Prevention

Join the Council and the Corporation for Supportive Housing beginning in January 2021 for a new opportunity for health centers serving people experiencing homelessness.

Participants will explore effective strategies to demonstrate that many promising diabetes prevention and self-management practices occur in settings other than the clinic. This learning collaborative will identify what housing case managers require to support continued diabetes prevention care and management, and provide training to address those needs. Topics will include nutrition, exercise, and medication management.

**Seeking Peer Reviewers for AMA?Journal of Ethics?Special Issue on Homelessness**

The call is still open for manuscripts for the November 2021 issue of the AMA Journal of Ethics, which consider questions related to the ethical implications related to health and homelessness. Submissions are due by December 23.?

Webinar Explores Food Insecurity and Trauma-Informed Approaches to BMI and Diabetes Care

Health centers and their affiliates recognize the necessity for trauma-informed care among individuals experiencing homelessness as it relates to food insecurity, body mass index screenings, and care. A Council webinar on Wednesday, December 9, from 1-2 p.m. ET, will explore food insecurity and its effects on individuals experiencing homelessness. Participants will discuss the various traumas related to homelessness and how trauma-informed care acts as a bridge to establish rapport and better serve the community.

New Brief Explores the Crucial Need for Comprehensive COVID-19 Testing and Services For People Without Homes

The collective understanding of COVID-19 is changing rapidly. Recent research suggests that using only symptom screening to isolate people who are potentially infected could overlook large numbers of asymptomatic COVID-19-infected individuals.

Our latest issue brief calls on public health authorities and emergency response systems in every community to conduct more comprehensive testing among people experiencing homelessness, and to ensure appropriate follow-up accommodations and supports.

New Resource Explores Using Community Information Exchanges to Coordinate Care for People Without Homes

Building and sustaining partnerships that address social determinants of health is crucial when providing care for people experiencing homelessness. Ensuring coordinated care is often challenging for partner organizations. Community Information Exchanges (CIEs) build on the work of the health care system's Health Information Exchanges and take the next step towards integrating data with other service organizations such as hospitals, health centers, housing providers, and additional community resources.

This publication highlights what CIEs are, how they can be used, and the benefits of CIEs during health care crises like COVID-19.

Tuesday''s COVID Coffee Chat to Explore Structuring Isolation & Quarantine Sites for People without Homes

Tuesday, May 5 | 12-1 p.m. CT

As the COVID-19 pandemic continues, many communities are seeking hotels and motels to use as isolation and quarantine sites for people experiencing homelessness. The County of Alameda, California, has initiated Project Roomkey, consisting of Operation Comfort, isolation housing for symptomatic or COVID-19 infected people experiencing homelessness, and Operation Safer Ground, safe housing for high-risk people experiencing homelessness.

In this webinar, presenters from Alameda County Health Care for the Homeless program will review their Project Roomkey Emergency Hotel Shelter Handbook and discuss the planning, implementation, and operations of Operation Comfort and Operation Safer Ground.

COVID-19 Resources from the Field: New Symptoms, Infection Prevalence Data, & More?

The CDC recently updated its list of symptoms for COVID-19, adding more symptoms to those already known-fever, dry cough, and shortness of breath. Reportedly appearing within 2-14 days, these symptoms can include:

Chills
Repeated shaking with chills
Muscle pain
Headache
Sore throat
New loss of taste or smell

Additionally, new research from the CDC''s Morbidity and Mortality Weekly Report?examines COVID-19 infection prevalence in homeless shelters. Drawing on test results from 1,192 residents and 313 staff members in 19 homeless shelters, the study''s findings suggest that male and African-American patients are being disporportionately impacted by COVID-19.

**New Piece in Annals of Family Medicine Explores "Housing as Health Care During and After the COVID-19 Crisis"**

The COVID-19 pandemic is vividly illustrating that housing is health care. Americans without homes are particularly vulnerable to COVID-19, and an outbreak among homeless populations is further threatening already burdened health care systems. A new publication in the Annals of Family Medicine?examines this situation, describing strategies to mitigate the impact of COVID-19 for homeless populations and for the health care system.

Co-authored by the Council''s Senior Director of Policy Dr. Barbara DiPietro, this article highlights medical respite care and larger structural changes needed to address COVID-19.

Upcoming BHTA Webinar: Cost Accounting for Behavioral Health Integration

April 30: 3-4:30 p.m. ET

Billing and reimbursement are two of the most significant barriers to behavioral health integration faced by health centers. The HRSA Center of Excellence for Behavioral Health Technical Assistance will host a webinar highlighting practical strategies that health centers at different levels of integration can use to understand the organizational level cost of delivering care across a range of integrated behavioral health models.

Council-Kaiser Permanente Partnership Awards $500,000 in Immediate Impact Grants

The Council was proud to partner with Kaiser Permanente (KP) to award half a million dollars in grants?to 16 nonprofits meeting the health needs of people without homes during the COVID-19 pandemic. Awardees in seven states and Washington, D.C., have received grants ranging from $25,000-$35,000.

Reminder: Help Us Collect Crucial Data on COVID-19 Testing for Homeless Populations

Have you been involved with homeless shelter or encampment testing events in your community? We need your help to inform national responses to COVID-19!?The Centers for Disease Control and Prevention and NHCHC have partnered to collect data on COVID-19 testing events among homeless populations. We aim to better understand the impact of COVID-19 on this vulnerable group and to inform future public health actions.

Homeless populations are often?overlooked?and?undercounted?in public health initiatives. We want to change that, but we need your data to advance an effective and humane response to COVID-19. Add one entry per universal testing event to our?data collection tool. Please also?forward the link?to others in your community who are also conducting broad testing in this population in order to obtain as much data as possible to inform future guidance.

Read Oral Histories of People Who Lost Their Lives in Homelessness: Life, Death, and the Streets in Between

From 2019-2020, the Council interviewed clinicians, administrators, and consumers of homeless health care to create the first national collection of oral histories of people who died while experiencing homelessness.

The resulting stories-15 lives spread across the United States-illuminate the perspectives and personalities of people who died without housing. Our hope is to illustrate not only their lives but the conditions that led to their deaths to reduce stigma, spotlight barriers to care, and bring attention to the preventable tragedy of homeless deaths.

The Council Welcomes New Communications Manager

Terri Woodmore has joined the Council as Communications Manager. She will work as part of the Advancement team to develop and coordinate our ongoing communications strategy and initiatives, and maintain high-quality communications including mass emails, newsletters, marketing materials, website design and content, social media, and audio/video packages. Terri has led communications efforts in previous roles at the Tennessee Primary Care Association and the Metropolitan Development and Housing Agency.

The Council is pleased to welcome Courtney Pladsen, DNP, FNP-BC, RN, as our new Clinical Director. Pladsen, a nurse practitioner at Greater Portland Health in Portland, Maine, currently serves as a subject matter expert for the Council on numerous topics and has served as a member of the Respite Care Providers Network Steering Committee.

As Clinical Director, Pladsen will lead initiatives to improve the delivery of quality primary care, and provide recommendations and training on emerging clinical issues affecting people experiencing homelessness.

"I've had the pleasure of working with the Council over the years," said Pladsen. "I deeply believe in the mission and vision of the Council and am excited to join this vital organization."

For more than 13 years, Pladsen has focused on providing high quality health care to under-resourced communities. She is leading an initiative to start the first medical respite program in Maine and is also a Fellow in the Robert Wood Johnson Foundation's Culture of Health Leaders Program (2018-2021).

"We are excited to have Courtney Pladsen join our team," said Darlene Jenkins, Senior Director of Programs. "She brings passion and dedication to address the inequities and injustices that exist in our society, and we look forward to her applying her knowledge, experience, and expertise to expand our clinical initiatives."

Pladsen earned her doctorate of nursing practice and master of nursing degrees from Georgetown University, and a bachelor of science in nursing from Boston College. Her appointment as Clinical Director will begin August 6.

National Leader Added to NIMRC Advisory Panel

The recently launched National Institute for Medical Respite Care (NIMRC) has announced that Pablo Bravo, System Vice President of Community Health at CommonSpirit Health, will serve as a member of its Advisory Panel. In his role at CommonSpirit Health, Bravo is responsible for the overall leadership and management of community health programs.

"We are honored to have Pablo Bravo contribute his experience and perspective to NIMRC," said Bobby Watts, CEO of the National Health Care for the Homeless Council and director of the NIMRC initiative. "The need for medical respite care is growing, particularly as the nation continues to confront the COVID-19 pandemic. The Advisory Panel will play a key role in shaping NIMRC and supporting our efforts to expand programs and increase access to care."

New Issue Brief Explores COVID-19, Medical Respite Care, & Alternate Care Sites

?
In response to COVID-19, communities are quickly creating Alternate Care Sites (ACS) for people without homes so they have a safe place to protect themselves from infection, await test results, and/or recover from the disease. These programs have the same purpose as medical respite care, which provides acute and post-acute care for unstably housed patients who are ready for hospital discharge but are too frail to recover on the streets or in shelters.

Our new brief aims to provide public health authorities, emergency response systems, and ACS program administrators with an operational framework to improve the quality of care at ACS programs and promote longer-term stability for vulnerable people. Ideally, communities can retain this increased capacity for medical respite care after the pandemic subsides given the high level of need for these programs prior to COVID-19.?

Council and NAEH Launching New COVID-19 Learning Series

Homeless response systems are facing unprecedented challenges in the wake of COVID-19. To train and support staff, the Council collaborated with the National Alliance to End Homelessness to develop the COVID-19 Online Learning Series. New Visual Guide Offers Consumer-Focused Tips for Wearing and Caring for Cloth Masks

The Centers for Disease Control and Prevention recommends covering your face in public during the COVID-19 pandemic, particularly in places where it is difficult or impossible to maintain social distancing. To assist patients experiencing homelessness, the NHCHC has created a new quick visual guide on how consumers can make, use, and clean cloth face coverings.

NHCHC Launches NIMRC on

July 15, 2020?

On July 15, the Council will introduce a new initiative as one of its top three strategic priorities ("to expand medical respite care") with the launch of NIMRC -- the National Institute for Medical Respite Care. Medical respite care, also known as recuperative care, has long been part of the Council and, by introducing NIMRC to a national audience of health care organizations, public policy leaders, and the general public, we believe this is a powerful new vehicle to facilitate the expansion of medical respite care across the nation.

Bobby Watts will begin directing NIMRC as a special initiative of the Council during a 24-month standing-up phase, with Julia Dobbins (Director of Medical Respite Care at the Council) heading NIMRC''s programs and services. Barbara DiPietro (Senior Director of Policy) and Dott Freeman (Senior Director of Advancement) will, respectively, head NIMRC''s public policy and fundraising. Additional team members will be recruited in the near future.

The NIMRC logo mirrors the Council''s logo and features the color "respite blue" -- the historical color of the Respite Care Providers Network. The RCPN will advise and counsel NIMRC as NIMRC advances best practices, delivers expert consulting services, and disseminates state-of-the-field knowledge by drawing upon the Council''s staff, subject matter experts, and consulting teams to provide training, technical support, research, and state and federal policy advocacy.

Information about the Institute will be available at www.nimrc.org beginning July 15, 2020.

New! Issue Brief on Universal Testing Strategies for Homeless Populations

This issue brief complements the?most recent CDC guidance?on testing and provides public health authorities, emergency response systems, HCH programs, and other community providers with feedback about these experiences. It also offers lessons learned and strategies for conducting successful universal testing events among homeless populations. The goal of this brief is to inform subsequent local testing events with new federal guidance, as well as feedback from the HCH focus groups on what has (and has not) worked well in individual communities.

The Centers for Disease Control and Prevention (CDC) and the National HCH Council are partnering to collect data on COVID-19 testing events among homeless populations.?Descriptive data and results of testing events are now available on our dashboard. View data representing?testing events, numbers tested, and positivity by state;?shelter types where testing occurred; whether testing occurred in response to reported cases; and testing information by race and ethnicity.

Data collection is ongoing. If you have conducted a universal testing event and your site is not represented on the dashboard, please add to the?data collection tool here! Together, we aim to better understand the impact of COVID-19 on this vulnerable group and to inform future public health actions.

Honoring the Life of Homeless Advocate and Friend, Paul Behler

The Council mourns Paul Behler-a fierce advocate for and a friend to many in the HCH community-who was killed recently at a bus stop in downtown Baltimore. Like those he fought for, Paul was experiencing homelessness on the streets. Remembered by his friends as a passionate seeker of social justice and systemic change, his passing is a reminder that we must do more to ensure affordable housing and health care for everyone and continue the fight to end the needless tragedies of homelessness and homeless death.?

Introducing Our COVID-19 Data Dashboard for Health Care for the Homeless

As HRSA collects data on how COVID-19 is impacting health centers, the Council is? extracting and processing this data to highlight how health centers serving patients experiencing housing instability, homelessness, and poverty are particularly impacted by, and responding to, the pandemic.

Our new COVID-19 Data Dashboard features comparative data on testing results, testing capacity, clinic closures, telehealth visits, and virus disparity by race and ethnicity from health centers, health centers with only 330(h) funding, and all health centers with 330(h) funding. More national health center data is available on HRSA''s website. New health center COVID-19 data is collected weekly, and we will continue to update these figures as new data emerges.

Informing Science: COVID-19 Testing Among Homeless Populations

Have you been involved with homeless shelter or encampment testing events in your local community? We need your help to inform the national response to COVID-19!?The Centers for Disease Control and Prevention (CDC) and National HCH Council are partnering to collect data on COVID-19 testing events among homeless populations. We aim to better understand the impact of COVID-19 on this vulnerable group and to inform future public health actions.

People without homes are often?overlooked?and?undercounted?in public health initiatives. We want to change that, but we need your data to advance an effective and humane response to COVID-19. Add one entry per universal testing event to our data collection tool. Please also?forward the link to others in your community who are also conducting broad testing in this population in order to obtain as much data as possible to inform future guidance.

Providing Medical Respite for People Experiencing Homelessness During COVID-19

Tuesday, August 11, 12-1:30 CT

Medical respite centers have played a critical role in supporting individuals experiencing homelessness, as well as hospitals experiencing a surge in capacity, during the COVID-19 pandemic.?On Tuesday, August 11, from noon-1:30 p.m. CT, a webinar coordinated by the Center for Health Care Strategies and the National Health Care for the Homeless Council will highlight the experiences and innovations of three medical respite centers in California and Washington state.?The webinar is made possible through support from the California Health Care Foundation.

Presenters will provide an overview of their programs and share how their medical respite centers responded to COVID-19, including changes to program design, procedures, and community partners, as well as lessons for improving their services for people experiencing homelessness.

The Council Seeks Medical Respite Coordinator

We''re looking to expand our medical respite team! The Medical Respite Coordinator will work with the Director of Medical Respite on projects of the National Institute for Medical Respite Care (NIMRC) and the Respite Care Providers' Network (RCPN).?Job responsibilities will involve completing pre-defined projects scoping the areas of technical assistance, education, training, and research, as assigned. Completion of these tasks will involve regular communication with the RCPN/NIMRC community to gather information and resources needed to execute this work well.

The National Institute for Medical Respite Care (NIMRC) Has Launched!

Today -- July 15, 2020 -- the Council launched its special initiative, the National Institute for Medical Respite Care (NIMRC), a first-of-its kind institute designed to expand the field of medical respite care and enhance knowledge about these vital programs. NIMRC has assembled a distinguished group of national health care leaders to serve on its Advisory Panel, in coordination with the leadership of the Respite Care Providers' Network Steering Committee.

Members of the Advisory Panel, all external leaders who are strong proponents of medical respite care, will support NIMRC during its two-year start-up phase. Initial members of the Advisory Panel are Dr. Stephen Cha, Chief Medical Officer for UnitedHealthcare Community & State; Dr. Sarita Mohanty, Vice President, Care Coordination of Medicaid and Vulnerable Populations for Kaiser Permanente; Dr. Pamela Riley, Medical Director for the District of Columbia Department of Health Care Finance; and Dr. Michelle Schneidermann, Director of High-Value Care for the California Health Care Foundation.

The Respite Care Providers Network (RCPN), the national voice for those who are too ill or too frail to recover on the streets or in a shelter, will provide counsel to NIMRC on direct care issues. The mission of the RCPN is to improve the health status of individuals by supporting programs that deliver medical respite and related services.

Facing a Dual Threat: COVID-19 and Extreme Heat

People experiencing unsheltered homelessness are particularly vulnerable to extreme temperatures and heat-related illnesses. Finding relief from the heat proves even more challenging and dangerous for people without homes during a pandemic when social distancing is recommended.?

As vaccines for COVID-19 are approved and become available for distribution, there are many questions about when they will be available?for people experiencing homelessness and for staff working in community-based programs, such as health centers and homeless shelters. While the development and distribution of the vaccines are crucial milestones in protecting people from future COVID-19 infections, significant challenges remain.?

The National Health Care for the Homeless Council recently issued an issue brief, "COVID-19 and the HCH Community: Vaccines for Patients and Staff," which is intended to: 1) assess the initial state vaccination plans as they relate to people experiencing homelessness; 2) identify the key factors for successful vaccination campaigns; and 3) identify actions the larger homeless services community needs to take.?

The Council also hosted a webinar?on December 18 that discussed vaccines for clients and staff. Panelists discussed the main challenges to vaccinating the HCH community; strategies needed to ensure a successful vaccination campaign; and immediate action steps to consider in order to be prepared.

Homeless Persons'' Memorial Day Observance and New Toolkit Shine Light on Lives Lost

"With each name read, with each life that you remember, let us resolve to make the situation right so that this is one of the last Homeless Persons'' Memorial Day observances we will ever need." -- Bobby Watts, NHCHC CEO

Nearly 200 people participated in the nationwide virtual observance of the 30th annual Homeless Persons'' Memorial Day on December 21. The event featured music and poems, and a listing and reflections of those who were lost in 2020. The observance also included remarks from representatives of the Council''s Homeless Mortality Data Workgroup.

In 2019, the Council initiated the Homeless Mortality Data Workgroup to unite expertise on homeless mortality reviews across the county. The Workgroup has compiled the Homeless Mortality Toolkit to share findings and implications from the communities across the country that conduct homeless mortality reviews, and provide guidance on developing or improving homeless mortality reviews. Lessons include how to create partnerships, methods of data collection and analysis, and examples of how communities have used this information to address homeless mortality.

Apply Now for Upcoming Learning Collaboratives

Two new learning collaboratives are currently accepting applications.

Beginning in January, the Corporation for Supportive Housing will create an opportunity for health centers serving people experiencing homelessness to explore effective strategies to demonstrate that many promising diabetes prevention and self-management practices occur in settings other than the clinic. This learning collaborative will identify what housing case managers require to support continued diabetes prevention care and management and provide training to address those needs. Topics will include nutrition, exercise, and medication management. The application deadline is Thursday, December 31. Learn more and apply here.

The Council is launching a learning collaborative in February on "Screening and Addressing Social Determinants of Health." At every session, a presentation will be given by an expert at the forefront of care. Topics will include the impact of SDOH on unhoused communities; the benefits of screener utilization; risk stratification of an SDOH screener tool; and the workforce and SDOH. This is a three-year project to ensure that participants have the support needed as they implement a new model. Updated Medical Respite Organizational Self-Assessment Now Available Online

The Medical Respite Organizational Self-Assessment is a newly updated tool that helps medical respite programs evaluate their alignment with the Medical Respite Standards, and guides quality improvement and goal setting for programs. The self-assessment tool can be downloaded here, or can be completed online here.?

New Council Webinar: "How to Conduct a Root Cause Analysis for Diabetes Prevention"

Registration is now open for an upcoming training session that will demonstrate different methods of conducting root cause analysis, tips and tricks along the way, and a simplified prioritization matrix to strategize which root causes and solutions should be priorities.

The webinar will take place on Thursday, January 7, 2021, from 9:30-11 a.m. Central and will be led by Kristen Bigelow-Talbert, Community Health Quality Manager at Bi-State Primary Care Association.?

Deadline Extended for Virtual Care Innovation Network

The Virtual Care Innovation Network, a community health collaboration founded by Kaiser Permanente, aims to bring together safety net organizations across nine states to redesign care so that virtual care models continue after the COVID-19 pandemic abates and beyond.

The program will enable participants to learn from peers and experts; test new approaches; accelerate the work they have already started; and develop ways to sustain virtual care as an essential component for how care is delivered into the future.?
